Context and commentary on verse 8.13
The sentence of the twelfth verse is completed here. Calling Om ‘the one-syllabled Brahman’ is an old tradition of the Upanishads. The link with the Katha Upanishad noted at the eleventh verse becomes clear here.
For Shankara Om is the name of Brahman: uttering it, one should remember the Lord whom it signifies. On ‘leaving the body’ he adds a fine note: departure happens by leaving the body, not by any destruction of the self’s own nature.
For Ramanuja the ‘supreme goal’ here is that of the seeker of kaivalya: the self freed from prakriti, similar in form to the Lord, from which there is no return. The path of the wise one who seeks the Lord himself, in his division, is given separately in the next verse. Madhva comments on verses twelve and thirteen together, and the gist is on the previous verse’s page.

In many homes there is a custom of saying ‘Om’ or the Lord’s name into the ear of someone in their last moments. One could say the memory of this verse lies behind it.
Hospitals too can make room for such wishes. If the family wishes it, letting them sit near the patient in the last hours and say a prayer softly is part of good care. Those who work in palliative care say that some people may keep their hearing until the end, even when they no longer respond. So let the words spoken at the bedside be gentle.
