An actor will give, a brahmin will give, and a Pashupata ascetic too will give if he is properly entreated. But the physician, who is himself the one always asking, how will he give? He is the man who is bent on taking even from one on the point of death.
About this subhashita
All three named live on what others hand them: the actor on a patron, the brahmin on dakshina, the Pashupata on alms. The point of the list is that it is the empty-handed who give.
Between martukamat and hartukamah there is a difference of one letter: the one who is about to die, the one who is bent on taking. The whole verdict of the verse sits in that one letter.
Yacamanah is in the nominative, so it describes the physician himself: the one who is asking. Some read it instead as though he were the one being asked. Set against samprarthitah, a passive form, in the line before, this active form looks deliberate.

For life today
The verse is not against taking a fee. But where saying no to the one who asks becomes the habit, what the profession loses is not money, it is trust.
There is something that stands against this verse too. In every age there has been a line of physicians who treat the poor free of charge, quietly and without announcing it. A whole section of verses of abuse exists only because that line exists.
