Context and commentary on verse 5.27
This verse and the next form one sentence. Shankara says here is the first outline of the yoga of meditation that the sixth chapter will expand. ‘Keeping them outside’ does not mean shutting eyes and ears. It means not dwelling again and again on the sounds and other objects that have come in through the senses.
About the gaze, Sridhara Swami gives a practical tip. If the eyes are fully closed, the mind dissolves into sleep; if fully open, it spreads outward. So the gaze rests between the eyebrows with the eyes half closed. Ramanuja glosses it here as placing the gaze at the tip of the nose. Madhva quotes a saying: ‘at the tip of the nose, or between the eyebrows’.
There is a difference of meaning in ‘making the breaths even’. For Ramanuja it means keeping the incoming and outgoing breath at an even pace. For Madhva it means resting in kumbhaka, the held breath. Ramanuja also adds that this is to be done seated on a seat fit for yoga, with the body upright.

Many people find that slow, even breathing calms the mind, and anyone can try it safely in a stressful moment. Its simplest form is to give the out-breath as much time as the in-breath.
But practices such as kumbhaka, holding the breath for long, may not suit those with heart or lung disease, uncontrolled blood pressure, or pregnancy. They would do well to proceed under an experienced teacher and after asking their own doctor.
