O lord of beings, is not this one deed of supreme help enough? Seeing the hosts of moving and unmoving beings within your belly and those standing outside, and in order to protect them all, you laid in your throat the poison that made every immortal flee, the poison that raised great flames and terrified all. You neither swallowed it nor spat it out.
Context and commentary on verse 31
Verses 29 and 30 spoke of the service the poet might render. This verse turns to something Shiva has already done. Behind it stands the churning of the ocean: when the gods and the asuras together churned the sea, the poison called halahala rose before the nectar did. The account appears in several Puranas with small differences, so we name none of them here. That Shiva accepted the poison when everyone else ran is the one part all the tellings share.
The heart of the verse is in the throat. Kukshigata means those who are inside his own belly, since the Shaiva position is that all the worlds abide in Shiva. Bahyasthita means those who stood outside and watched. Had he swallowed the poison, harm would come to those within; had he spat it out, harm would come to those outside. So it stopped in the throat. Pashyan, seeing, and rakshitum, in order to protect, are the two words that keep this reason inside the verse itself.
There is a wry smile inside the compound sarvamartyapalayanaushadha. Amartya is one who does not die, that is, a god. Palayana is running away. Aushadha is medicine. Together they call the poison the medicine for making every god take flight. The churning was for nectar, and what came first to hand was poison, and the deathless ones ran from it. The whole weight of the verse rests on two negatives, na gilitam and na udgirnam, not swallowed and not thrown out.
A word about the poison. Halahala is an image from the Puranas, and what this verse speaks of is Shiva’s compassion. Reciting a verse is not a remedy for poisoning in real life, where immediate medical help is what is needed. Tradition holds that the mark left in the throat gave Shiva the name Nilakantha, the blue-throated, and the next verse takes up that very mark. The metre is shardulavikridita, a quarter of nineteen syllables with a pause after the twelfth.

In a hospital lift people stand without speaking. Someone who knows a hard piece of news finds that at that moment it can neither be let out nor swallowed down.
Such moments come to us too, at home and at work. Something that will hurt one person if it is said and another if it is not has to be held for a while. The throat of the one who holds it does not turn blue, but that person knows the weight of it.
