Those who do not know the true sense of the science and are given over to the knife alone: such physician-nooses one should keep clear of from a long way off, exactly as one keeps clear of the nooses of Yama.
About this subhashita
Shastra and shastra, the science and the knife, are one long vowel apart. The verse sets them side by side in the first line. The reader hears the pair before the argument reaches him.
Pasha means a noose. Coming at the end of a compound it also carries plain contempt, much as haalu does in Kannada. The verse uses bhishakpasha in that sense and then, at once, uses the same word pasha in its literal sense as Yama’s noose.
The pair tyajet durat matters. It is not only refuse, it is do not even go near.

For life today
Everyone feels the pull towards the treatment one happens to be good at rather than the one that fits the patient’s problem. Anyone teaching residents meets this lesson again and again.
The verse is not against surgery. It is against the knife without the science. In the hand of someone who knows when it is needed, that same knife saves a life.
